How do I qualify for a Utah Medical Marijuana Card?

You can start the process now. Take 20 minutes to apply, just complete steps 1-4.
If you don’t qualify, there’s no charge. 

We do not accept patients under 21, sorry.

 

1. Remember that not all health conditions qualify, so be sure to detail the history of your qualifying health condition on our intake form which you can complete here. ‘Anxiety’ for example, by itself, won’t qualify. Call us if you have multiple health issues or are unsure whether these conditions apply to you.

Utah Qualifying Health Conditions are as follows:

  • Alzheimer’s disease
  • Pain lasting longer than two weeks that is not adequately managed, in the qualified medical provider’s opinion, despite treatment attempts using conventional medications other than opioids or opiates or physical interventions.
  • Amyotrophic lateral sclerosis (ALS)
  • Persistent nausea that is not significantly responsive to traditional treatment, except for nausea related to:
    • Pregnancy
    • Cannabis-induced cyclical vomiting syndrome
    • Cannabinoid hyperemesis syndrome
  • Crohn’s disease or ulcerative colitis
  • Epilepsy or debilitating seizures
  • Multiple sclerosis or persistent and debilitating muscle spasms
  • Post-traumatic stress disorder (PTSD) that is being treated and monitored by a licensed health therapist (defined here), and that:
    • Has been diagnosed by a healthcare provider by the Veterans Administration and documented in the patient’s record; or
    • Has been diagnosed or confirmed by evaluation from a psychiatrist, doctorate psychologist, a doctorate licensed clinical social worker, or a psychiatric APRN
    • Please Note: The key medical record requirement with PTSD is to get a letter from your current therapist (yes actively participating in therapy is a requirement due to data showing improved outcomes) stating that you have PTSD or symptoms of PTSD.
  • Autism
  • A terminal illness when the patient’s life expectancy is less than six months
  • HIV or acquired immune deficiency syndrome
  • A condition resulting in the individual receiving hospice care
  • Cancer
  • Cachexia
  • A rare condition or disease that affects less than 200,000 individuals in the U.S., as defined in federal law, and that is not adequately managed despite treatment attempts using conventional medications (other than opioids or opiates) or physical interventions
  • A condition that the Compassionate Use Board approves (once established) on a case-by-case basis

2. Make an appointment online or with a phone call to our intake team at 801-448-9990. New patients are seen now seen Monday through Thursdays in Lehi with select weekend times in Cedar City. You’ll get an appointment confirmation email within 24 hours including the links you need to begin your application for a Utah med card. Please be sure your intake form gets completed and that you are ‘awaiting certification’ at evs.utah.gov.

3. Read our confirmation email completely! It includes everything you need to apply for Utah Medical Cannabis. Complete an intake form via our easy patient portal. List all current prescriptions for your medical marijuana pharmacist, as well as any previous injuries, diagnoses, or symptoms you are currently experiencing.

4. Create a Utah.gov login ID. Then login to apply for medical cannabis. If you get confused, consult the Patient Guide.

​5. Gather your health records to document any diagnoses, injuries, or treatments of qualifed conditions. Gather records of any injuries, procedures, or conditions that may have led to chronic pain if you are seeking pain relief with medical cannabis. Start this process right away. Call us with any questions. Medical records are NOT a requirement in some cases but should be presented whenever practical to assist us in understanding your health conditions.

6. Come see us at our beautiful new home in the Spaces Building in Lehi. Think of some health goals you’d like to achieve with medical cannabis and write them down.

​7. Log back into the EVS system and pay the $15 Utah State Medical Card Fee. We can help you do this at the time of your appointment.

​8. Log back to the EVS system after a week or so to print your card, and enjoy your legal cannabis access at a Utah pharmacy.
